Retiree United Healthcare Medicare Advantage Plan 65+
UnitedHealthcare® Group Medicare Advantage (PPO) Plan
Medicare eligible Retirees and/or covered dependents who reach Medicare eligibility either due to age or disability will not have the option of remaining on the City group medical plan. Retirees who reach age 65 or disability must elect both parts A and B of Medicare to enroll in one of the two United Healthcare Medicare Advantage Plans (In Network or Out of Network) offered by and subsidized by the City.
The City of Savannah is partnered with UnitedHealthcare® Group Medicare Advantage (PPO) plan for all Medicare-eligible retirees and their Medicare-eligible dependents for health care coverage. A plan custom designed exclusively for City of Savannah to help ensure retirees have the same benefits as in years past.
The UnitedHealthcare® Group Medicare Advantage (PPO) plan is a Medicare Advantage Plan that delivers all the benefits of Original Medicare (Parts A and B), includes prescription drug coverage (Part D) and offers additional benefits and features. This plan is not a supplement plan and does not pay secondary to Medicare.
Submit ALL claims directly to UnitedHealthcare for payment, not Medicare.

- See any provider (in or out-of-network) at the same cost share, as long as not opted out of or been excluded from Medicare.

Important Information
Retiree must be entitled to Medicare Part A and enrolled in Medicare Part B. The Medicare Part B premium must be paid by the Retiree to be eligible for coverage under the UnitedHealthcare® Group Medicare Advantage (PPO) plan.
*If Retiree choose to join a different Medicare plan, contact 800-MEDICARE anytime, 24 hours a day, 7 days a week, for help in learning how. TTY users can call 877.486.2048. The state in which retiree is a resident may have counseling services through the State Health Insurance Assistance Program (SHIP). They can provide personalized counseling and assistance when selecting a plan, including Medicare Supplement plans, Medicare Advantage plans and prescription drug plans. They can also help find medical assistance through state Medicaid program and the Medicare Savings Program*
