SAV4Health Incentive Program
The City of Savannah strives to offer competitive health benefits for employees and their dependents. The SAV4Health Incentive program encourages employees to complete the wellness incentive program each year to earn $1,000 in savings on medical premiums for the next plan year.
HOW TO EARN SAV4HEALTH INCENTIVE
All City employees on the City of Savannah Health Plan must complete four (4) Wellness Activities by November 15th to earn $1,000 savings on medical premium rates for the next plan year.
Employees hired after July 1st will receive the SAV4Health rate the following year. The wellness activities must be completed by November 15th the following calendar to maintain the SAV4Health premium incentive reduced rates.
INSTRUCTIONS: How To Complete Your Wellness Activities
COMPLETE FOUR WELLNESS ACTIVITIES
Step 1: Have an Annual Primary Care Physician (PCP) Visit with Biometric Screening Lab Work Completed (2 options)
- Take the Biometrics Screening Form to your appointment
- Attend a Biometric Screening Event. To schedule your appointment, register online or call 912-819-8177
Choose one of the three options to complete Step # 2, 3, and 4

- Scan: QR code to the Quantum Health website log on
Step 2: Report Biometrics Results from PCP Visit or Biometric Screening Event
- Self-report your biometric screening results
- Record the date of the PCP visit or attendance at the biometric screening event
- Follow the How to Complete Your Wellness Activities for assistance
Step 3: Complete Wellness Assessment (Health Questionnaire)
- Answer questions about lifestyle and nutrition
- Approximately 20 minutes to complete
Step 4: Complete Tobacco Affidavit (2 Options)
- Certify you are tobacco-free, activity is complete
- Certify you are not tobacco-free, enroll and complete a tobacco quit program
- To gain credit, enroll and complete a tobacco quit program (cost covered at 100% by the City). See the Tobacco Cessation information HERE.
- Without enrolling in a tobacco quit program, you will not qualify for the SAV4Health incentive, and you will be charged the standard (full) rate for your medical plan.
